Private Geriatric Care Management

Our experienced care managers are well qualified to assist seniors and caregivers in identifying and accessing the services seniors need to live with dignity in their own homes for as long as possible.

A multidisciplinary team evaluates the type and nature of services that will meet each individual senior’s needs and

• Working with the senior, the caregiver, and the physician ensures that each individualized plan of care addresses critical needs

• Locates and coordinates services that meet those needs

• Addresses problems as they arise, relieving the caregiver or out of town relatives of this burden

• Calls and visits each senior at home or in a care facility to monitor the quality of services being provided.

• Provide peace of mind for out of town relatives concerned about the safety and well-being of their loved one

• Supports caregivers through sharing information and offering educational opportunities on a regular basis

• Provides referral for needed caregiver and senior support

This is a fee-based service.
