The
Garden Bridge Program is a unique support program for seniors in the early
stages of cognitive failure and their caregivers. The program combines
independent and supervised activities that ensure the senior continues to
enjoy life. A senior care professional supervises the program and an
interdisciplinary team works with the caregiver and the senior to ensure that
there is an individualized plan for care. Participants are provided with a
companion throughout their time at the Center, participate in a wide range of
activities including educational programs, studio art, discussions, and
memory classes and exercises overseen by experts in cognitive disorders. There
are individual activities and small group sessions.
The program also includes

Caregiver
support including seminars on senior issues and needs held at a time
that caregivers can easily attend, the Bridge caregiver support group,
individual support from the Bridge Program coordinator, and referral for
additional support.

Monthly support sessions with the seniors
and their caregivers to discuss program participation, senior
satisfaction levels, caregiver needs, status of the senior’s condition, and
other issues relevant to providing appropriate care
The Garden Bridge Program is a fee-based
program (fees are available on request.) Donations for the Garden Bridge
Scholarship Fund are welcomed.
